One Life to Live: The finale of the venerable daytime drama recalls important moments from the series (1 p.m. ABC).

Chuck:
While Chuck and Sarah (Zachary Levi, Yvonne Strahovski) attempt to settle down, Morgan’s (Joshua Gomez) past as the requires them to undertake one last mission, on which they get help from Bo Derek (guest starring as herself). Adam Baldwin also stars in this new episode (8 p.m. NBC).

Extreme Makeover: Home Edition:
The charitable home improvement series ends its run with back-to-back episodes that involve building new homes for seven families in Joplin, Mo., which was devastated by a tornado in May (8 and 9 p.m. ABC).

CSI: NY:
When Mac (Gary Sinise) and his colleagues investigate what looks like a simple home invasion, the perfect facade of the victim’s family begins to crumble in this new episode. Sela Ward and Carmine Giovinazzo also star (9 p.m. CBS).

Supernatural:
After he and Sam (Jared Padalecki) tangle with the God of Time (Jason Dohring), Dean (Jensen Ackles) is sent back to 1944, where he is arrested by Eliot Ness (Nicholas Lea) in this new episode (9 p.m. KTLA).

Great Performances:
In the new production “Let Me Down Easy,” Anna Deavere Smith offers indelible portraits of individuals confronting the price and politics of health (9 p.m. KOCE).

Sweet Home Alabama:
City slickers and country folk vie for the affections of a bachelor or bachelorette as the unscripted series returns (9 p.m. CMT).

Blue Bloods:
Danny and Jackie (Donnie Wahlberg, Jennifer Esposito) investigate when a witness to a murder at a diner claims to have seen a uniformed officer fleeing the scene. Tom Selleck and Bridget Moynahan also star (10 p.m. CBS).
